  • Please use this space to describe any allergy information, dietary requirements, or if you child has contact with any other professionals such as social worker, health visitor, speech and language therapist, etc, please give their contact details. You should also include any information regarding Special Educational Needs and Disabilities so we can determine how best to support you child.

  • From time to time we like to take photographs and video footage of your child/ren in school. This enables us to keep a record of their achievements and to celebrate the children’s work. The Early Years Advisory Teachers who visit our school would also like permission to take photos of your child in natural play situations. These photographs and videos could be used in our school, including on the school web site, or possibly for Early Years training and positive promotional purposes. If you have no objections to the above would you kindly indicate this by ticking the checkbox below.
  • Parent photography and videoing - Unless there are valid objections you are allowed to film/photograph your child in school performances/activities. However, due to issues of safeguarding, permission has not been granted to place these pictures/video’s on social media for public viewing, if it includes children other than your own.

    Please note - When your child starts school, would you please bring in their birth certificate so that we can verify your child’s age.
